Services for Elderly
Diagnostic Assessment
The Diagnostic Assessment is an intensive clinical and functional face-to-face evaluation of a recipient’s mental health, developmental disability, or substance abuse condition that results in the issuance of a Diagnostic/Assessment report. Recommendations from the assessment provide the basis for services and for the development of a Person Centered Plan (PCP). It also evaluates the recipient’s level of readiness and motivation to engage in treatment. Results from a Diagnostic/Assessment include an interpretation of the assessment information, appropriate case formulation and an order for immediate needs of the individual.
Home and Community Supports
Home and Community Supports meets the support needs of individuals within their own homes by providing instruction and assistance to enable the individual to acquire and maintain skills that will allow him/her to function with greater independence in the community.
Case Management
Case Management services are provided to clients for the purpose of assessing, arranging, linking, monitoring, and coordinating, services.
Additionally, case management provides intervention programs that are sensitive to the needs of the client and that respect the uniqueness of each person and family.
Personal Care Services
Personal Care Services provides support, supervision and engaging participation with eating, bathing, personal hygiene and other activities of daily living.
Supervised Living
Supervised living is a 24-hour facility which provides residential services to individuals in a home environment where the primary purpose of these services is the care, habilitation or rehabilitation of adults who have mental illness, a developmental disability or disabilities, and who require supervision when in the residence.
